The undersigned Animal Owner acknowledges that the Premises, and the animals, guests, items and activities thereon, pose dangers to people and the animals. Such dangers include, by way of example and not limitation, animals that can bite, kick, scratch, trip, knock down and fight, indoor and outdoor surfaces that may be slippery, equipment that can trip, persons who cannot control their animals, and activities involving animals that can result in injury form bites or other causes.

Accordingly, Animal Owner, on behalf of him/her, his/her spouse and minor children, an anyone else whom Animal Owner brings onto the Premises, waives any and all legal duties on the part of Pack Leader Plus or its Owners to keep the Premises free of dangers and the risk of injuries, losses, damages, costs and expenses by any means above described, and other injuries, losses, damages, costs and expenses of every possible cause and description unless inflicted intentionally or recklessly by the agents/ employees of Pack Leader Plus or its Owners. The Premises are operated by Pack Leader, Plus… BTZ LLC, a Missouri limited liability company.

This Agreement is binding upon Animal Owner, the spouse of Animal Owner, and the children of Animal Owner. Anyone entering the Premises must sign this Agreement. Animal Owner agrees to cause anyone that he or she brings on the Premises, (including his or her spouse, other adults, and for children not related to Animal Owner, their parents) to execute a copy of this Agreement, which will be made available by an onsite agent/employee of Pack Leader, Plus…. Should any such not sign this Agreement, Animal Owner will defend, hold harmless and indemnify Pack Leader, Plus…and its Owners, and their agents/employees, against any claims, demands, lawsuits, losses, costs or expenses, including attorneys fees, brought by or on behalf of any such persons. Animal Owner assumes the risks to and is liable for the actions of each animal they (or their agent) bring upon these Premises. If the animal is injured in a fight or an accident, gets fleas, ticks or worms, contracts any illness or disease, or engages in any unwanted behavior, Animal Owner accepts the risk of the same and agrees that Pack Leader, Plus…and its Owners, employees and agents shall not be held responsible for such occurrences or any resulting injuries, losses, damages, costs or expenses. Animal Owner agrees to pay all losses and damages resulting from any behavior of the animal, at any time, that causes property damage or personal injuries, including but not limited to bites, suffered or incurred by any person or another animal.

If the animal causes injury or loss to any property, person or another animal, Animal Owner will defend and indemnify Pack Leader, Plus…it’s Owners, agents and employees from any and resulting claims, demands, lawsuits, losses, costs or expenses, including attorney’s fees.

This Agreement supersedes all prior discussions, representations and agreements of the parties, and expresses the entire understanding between Animal Owner and Pack Leader, Plus…regarding the matters described above. All parties confirm that no other promises, representations or oral understanding have been made with regard thereto. This Agreement may be amended only by a written instrument signed by both Animal Owner and Pack Leader, Plus…Owners.

Any remedy provided in this Agreement is in addition to any and all other remedies provided by law or equity. All animals entering this property must be leashed, crated, or otherwise contained, restrained and controlled by the A emoved from any confinement without permission and attendance by Pack Leader, Plus… employees.

Absolutely NO dangerous, Illegal, or poisonous animals(s) are permitted anywhere upon the Premises. Any animal that displays aggressive tendencies towards any humans must be contained to predetermined areas and treated with high levels of caution. These animals will be considered Dangerous cases and Tom and Brooke Zeigler can evaluate if any services can be done. Cases will vary. Pack Leader, Plus… may refuse any services to a dog who cannot meet criteria of the Pack.

Animal Owner are required to complete the PET questionnaire for animal(s) brought upon premises.

I AM VOLUNTARILY BRINGING BY ANIMAL(S) TO PACK PEMF THERAPY FOR PEMF SESSIONS. I UNDERSTAND ALL OF THE RISKS ASSOCIATED WITH PEMF AND DO HEREBY CONSENT TO AND AUTHORIZE PEMF TO BE PROVIDED. I HAVE BEEN ADVISED BY BROOKE OR TOM ZEIGLER OF THE BENEFITS OF PEMF AND HOW PEMF IS PERFORMED. I UNDERSTAND FULLY THE NATURE AND EXTENT OF PEMF SESSIONS. I HEREBY WAIVE ANY CLAIMS AGAINST BROOKE AND TOM ZEIGLER AND PACK PEMF THERAPY; BTZ LLC ARISING OUT OF PEMF SESSIONS TO BE PROVIDED AND I RELEASE BROOKE AND TOM ZEIGLER AND PACK PEMF THERAPY; BTZ LLC FROM ANY AND ALL CLAIMS AND SUITS RELATED TO PEMF THERAPY. I HEREBY AGREE TO ASSUME ALL THE RISK OF INJURY OR ANY OTHER ADVERSE RESULT, INCLUDING DEATH, WHICH MAY BE ASSOCIATED WITH PEMF SESSIONS PROVIDED. BROOKE AND TOM ZEIGLER (PACK PEMF THERAPY: BTZ LLC) HAS ANSWERED ALL OF MY CONCERNS AND PROVIDED ALL THE INFORMATION REQUESTED IN CONNECTION WITH PEMF THERAPY TO BE PROVIDED.
